RECORD KEEPING

All our records are kept in accordance with the Data Protection Act of 1988, including your access to medical records.  All medical records are confidential.

Should you wish to view your records at any point, you should ask a receptionist to make an appointment for you to see your usual doctor.  Please make sure that the receptionist knows that the appointment is because you want to see your records.  A fee will be payable for this service if you require any copies of your records.

We will pass on your details to another health care organisation if we have to refer you to another service for health care investigations.  You will be aware of this referral.

Sometimes we are asked by the West Hertfordshire Primary Care Trust to supply them with data for statistical or audit purposes.  We always ensure that this data has been anonymised, so that it cannot be tracked to an individual patient.

 

We would like to make a note of patient email addresses in their records and would be grateful if you could give our receptionists this information.  We will only use this, with your consent, to advise any health issues and surgery news.

We will never disclose your email address to anyone outside the practice.
